Dental Clinic: Completed Patient Registration Packet (available in office or on website), photo ID, insurance card(s) School Dental Sealant Program: Consent form (available in office or on website form can also be mailed to home address)

Dental Clinic:

Provides a full range of dental services for local youth, including:

-- Exams (twice per year)

-- Cleanings

-- Fluoride treatments

-- Sealants

-- Repair of decayed teeth

-- Fillings

-- Some root canals

-- Stainless steel crowns

-- Space maintainers

School Dental Sealant Program:

Conducts visits to all public schools in the area served to provide school dental exams, cleanings, fluoride treatments, and sealants.
