Programming includes: - Medical case management. - Support groups. - Oral health care. - Housing services. - Utility support. - Primary medical care. - Nutrition services. - Vaccination programs. - Transportation assistance. - Mental health services. - Legal assistance (for eligible individuals within the care connect region).

- No fees. - Assistance programs available to access PrEP if uninsured or insurance does not cover full cost.

Eligibility

- Individuals living with HIV/AIDS. - Individuals who meet income and insurance guidelines are also eligible for dental care, housing services, utility support, primary HIV medical care, nutrition services, vaccinations, transportation assistance, mental health services and other services.
