May include: Copy of birth certificate Copy of social security card Immunization records Parent/guardian's last two paycheck stubs Parent/guardian's last year's income tax return Copy of two recent bills to show residency Current Medicaid card or Insurance card Copy of court order if mandated by court Psychiatric or psychological evaluations Substance abuse treatment records Mental health records Medical history Educational records, such as IEPs if applicable Child welfare and social service records

(1) Walk in during listed hours, Or call, email or fill out online contact form and indicate that you would like to receive services or make a referral for someone who does (2) A staff person will confidentially collect and screen your referral information , including relevant documentation (3) The admissions team will work with you to schedule your free assessment, to be conducted in person or over the phone (4) Those who have been assessed and approved for admission will be scheduled for an intake (5) Refer to Admission Checklist on website to find out what to documentation and other items to bring
