To qualify, a patient must have no health insurance OR their only health insurance is Illinois' Health Benefits for Immigrant Adults (HBIA) or Health Benefits for Immigrant Seniors (HBIS). Patient income must also be at or below 300% of Federal Poverty Level. CommunityHealth must be the only place where their patients will be receiving health care.
