Restricted to ages 60+; Must reside within program boundaries (Northside of Chicago and the Near North, Northwest, and Far North Suburbs); Must have liquid assets below $300,000 (If assets exceed the criteria, they will offer care management services on a fee for service basis)

Application process

Call (773) 508-1000 or contact them using the form online to find out more about in-person care management services. One of CJE's intake social workers will first conduct a scheduled phone interview to give preliminary recommendations and resource referrals. Finances will also be assessed for eligibility for the program. Once eligibility is determined, a care manager will follow up directly and an in-home visit will be scheduled to meet.
