Eligibility
- Must be seniors 60 years of age or older - Must be a U.S. citizen or legal permanent resident of Illinois - Must have non-exempt assets of $17,500 or less (excludes home, car or personal furnishings) - Must have an assessed need for long-term care (to be at risk for nursing facility placement as measured by the Determination of Need (DON) assessment).
