COMMUNITY SUPPORTS - SUPPORTED LIVING SERVICES | COMMUNITY CHOICES
Provides services to adults with intellectual and developmental disabilities. Helps manage DHS bureaucracy with families and connects Personal Support Workers (PSWs) with clients. Teaches 8-week classes on various life skills, including healthy relationships and sexuality, transportation, community and home safety, etc.
Self-Direction Assistance for People with the Home Based Support Waiver for People with I/DD:
