TRANSITIONAL, PERMANENT SUPPORTIVE HOUSING AND RAPID RE-HOUSING FOR FAMILIES | MERCY COMMUNITIES
Provides affordable housing and support services to those in need of help to build a new future. Offers a Transitional Living Program for up to two years to assist homeless young women and their children to achieve stability in their lives by providing them with a stable home and intensive support services. Also offers aftercare services to help families as they transition into permanent housing. Offers permanent affordable housing and supportive services for homeless families with dependent children in which the head of household or their child has a disability. The program enables families with severe and persistent issues, such as mental illness, chronic health problems, and substance abuse to achieve long-term housing stability. The goal is to help individuals reach their fullest potential by encouraging and mentoring them. Some of the services provided include: - Counseling. - Parenting classes. - Budgeting. - Support groups. - Job or vocational training.

Eligibility
- Clients enter the Transitional program after an intake based on need and unit availability. - Permanent Housing programs referrals are completed through the coordinated entry system process and referrals.
