Offers home visits by registered nurse or case manager to low-income pregnant women, families with infants up to age one, or high-risk children up to age two. Provides variety of advocacy services, which include: - Medical - Dental - Social Services - Counseling - Health Screenings - Referrals to Community Resources (health care providers, transportation, nutrition, and mental health services)
