PATHWAYS TO COMMUNITY LIVING | SENIOR SERVICES - A DIVISION OF EFFINGHAM CITY/COUNTY COMMITTEE ON AGING
Helps residents who are currently in long-term care facilities to move out of the nursing home and back into their community. Pathways to Community Living Program provides individuals choices about where they can live, as well as assistance with community supports and services.

Meets all of the following: - Has resided in a nursing home or intermediate care facility for at least 90 days. - Currently recieving or be eligible for Medicaid. - Interested in moving back to the community and must choose to do so. - Able to have their needs met appropriately and safely in the community. - Meets one of the following: - Over age 60. - Physical disabilites. - Mental illness. - Developmental/intellectual disabilites.
