The Helping Others Gain Apartment Re-Entry (H.O.G.A.R.) Program helps transition individuals with mental illnesses residing in Illinois nursing homes (Colbert Class members) directly to the community through intensive case management services.The program seeks to address the behavioral needs of individuals experiencing a chronic mental illness inadequately housed in nursing home settings, who can greatly benefit from intensive, community-based services that support improved independent functioning, self-sufficiency, and positive long-term health outcomes.
