Supported Living Services for Adults With Disabilities | Bridges Community Support Services, Inc.
Community Support: Part-time individualized service that focuses on increasing skills, promoting independence, maximizing self-determination and increasing participation in the community. Services may include help with budgeting, paying bills, meal planning, cooking, nutrition awareness, medical appointments, maintaining or searching for housing, home management, accessing public transportation, etc. Residential Services: Offers support up to 24 hours a day in a shared home or an individual home. Staff work with individuals to develop a yearly plan with goals and develop outcomes. Assistance may include help monitoring medications, attending doctors appointments, personal care, home making activities, social activities, strategies to compensate for cognitive, behavioral and physical challenges and more.
