Person at least 18 years of age who was born in DuPage County; Parent listed on the birth certificate of the child whose certificate is being requested; Legal guardian with certified court documentation proving guardianship; Legal representative (such as an attorney) acting on behalf of the person named on the certificate; Agent authorized by court ordered power of attorney.
