Open to those who meet all of the following: - Has resided in a nursing home or intermediate care facility for at least 90 days - Currently receiving or be eligible for Medicaid - Interested in moving back to the community and must choose to do so - Able to have their needs met appropriately and safely in the community - Meets one of the following: Over age 60, has physical disabilities and/or mental illness and/or developmental/intellectual disabilities
