(1) Visit the website to complete the online intake questionnaire, or (2) Call to schedule an appointment (3) If qualified, a referral will be provided to see an eye care specialist.

Must be a U.S. citizen or legal resident; Cannot belong to an HMO or have eye care benefits through the VA; Has not had an eye exam in 12 months or more;
