Child and Family Connections (CFCs) provide a single point of entry to the Illinois Early Intervention Program (EI). Early Intervention offers developmental support and case coordination services for families with children under 3 years old with developmental delays. Service Coordinators assist referred families through the process of enrollment into the EI, as well as initiate referrals for evaluations and direct services. Eligible participants receive case coordinator through CFC to monitor individualized goals, strategies, and services. Additionally, staff assist families with the transition from the EI program into local school/care special education services based on the family's priorities and the child's needs. CFCs also staffs a social-emotional consultant to provide coordinators and providers information, training, and consultations through reflective supervision; a parent liaison to contribute an understanding of the EI program through the lens of a parent with special needs; and a coordinator who acts as a liaison to community partners, providers, and referral sources.
