Chestnut Family Health Center is a community based health center that offers adult and pediatric primary care. This service provides a medical home for Chestnut's clients, especially those with co-morbid mental illness and/or substance use disorders. Services include: - Basic primary care, preventive care, and prescription services. - Screenings (cancer, communicable diseases, cholesterol, blood lead, pediatric vision, hearing, and dental). - Annual wellness exams for all ages. - Well child exams and immunizations. - Lab collections. - Obstetrical & gynecological care. - Physicals (school, sports, employment).
