Eligibility

- Resides in a nursing facility in Illinois. - Lives in Illinois. - A U.S. citizen. - Between the ages of 18-59. - Has a severe disability which will last for 12 months or the duration of life. - Has physician's approval of plan of care. - Has no more than $17,500 in non-exempt assets. - Receives or applies for Medicaid benefits.
