DIABETES CARE & MANAGEMENT PROGRAM | SOUTHERN ILLINOIS HEALTHCARE FOUNDATION

Helps patients manage their uncontrolled diabetes through access to care, specialists, and education. The program also offers multiple ways for patients to track diabetes by using a continuous glucose monitor system which tracks patients glucose levels throughout the day, and can alert patients if levels are too high or low.

- Patients with hospitalizations and ER visits related to uncontrolled Diabetes. - Patients who have Medicaid (WellCare or Meridian) insurance. - Patients who reside in the Metro East area.
