Application process
Call Community Action Partnership to schedule an appointment. Annual recertification required. Documents required:
  • Photo ID
  • Proof of income for the last 30 days
  • Social Security cards for all household members
  • Current utility bill
  • Copy of rental agreement (if renting) showing that utilities are included, the monthly rental amount, and landlord contact information
  • Proof of benefits if receiving assistance from the Illinois Department of Human Services
