Provides the following in-office and outreach services: - Information and assistance in applying for benefits such as the BAA program, Medicare, Medicaid, and other insurance. - Needs assessments and referral follow-ups. - Assistance with access to other agencies. - Assistance to caregivers and grandparents raising grandchildren.
