Mental Health Pre-Admission Screening

HSI provides screenings and assessments for persons with severe mental illness who are seeking admission to public aid-funded nursing home facilities and/or need specialized rehabilitative services. Resident Review and Targeted Case Management are set up to review nursing home placements and readiness for change to different levels of care and case management support services.
