Call for an intake interview Documents Required (varies, on a case-by-case basis): - Photo ID for all adults in the household - Social Security Cards for all household members, including children - Proof of income for all household members for the last 30-days prior to application date - Proof of lease agreement - Eviction or late notice, if applicable

- Must be a Lake County resident - Must meet income requirement: 200% of Federal Poverty Guidelines - Tenants living in subsidized housing or with a Section-8 voucher are not eligible
