Transitional age program provides assistance for those youth and young adults transitioning to independent living, including subsidized housing, help with medications, budgeting, keeping house, etc. Transition age youth may be planning to transition out of foster care or their parents' home, or they may already be living on their own but need extra help to maintain their successful independence
