Eligibility
Must be under 17 years of age; Must be in need of specialized, out-of-state medical care; Parents/legal guardians travelling with the child; Must meet household income guidelines.
Required documents
Documentation of confirmed appointments from patient's treating medical professional, such as an MD, DO, or PA-C
Proof of income (IRS tax documents for all income earners in household, SSI and/or SSDI statements)
Additional documents may be required.
