Application process
Individuals who need permanent housing through Facing Forward must be referred through Chicago's coordinated entry system. To be eligible for housing, applicants must have documentation of a disability as well as proof of homelessness for at least 1 full year. To access this service, individuals must first receive a referral to Facing Forward, then the staff will contact the individual to set up an interview.
