Application process

Individuals who need permanent housing through Facing Forward must be referred through Chicago's coordinated entry system. To be eligible for housing, applicants must have documentation of a disability and proof of homelessness for at least 1 full year. To access this service, individuals must first receive a referral to Facing Forward, and then the staff will contact them to set up an interview. Every household is matched with a dedicated case manager who walks alongside them as they work toward their goals. With personalized support and resources, clients can improve their health, strengthen life skills, and pursue education and employment opportunities that empower them for lasting success.
