- Photo ID - Social Security cards for all members of household - Documentation of need - Proof of Income (30 days) for all households members and/or Zero Income affidavits for all member over 18 years of age - Proof of monthly household expenses (examples: mortgage or rent, gas, electric, water, sewer, trash, phone [cell]) - Car Payment - Furniture or Appliance Payment - Eviction or Late Notice - Appliance Estimate - Furnace Repair Estimate - Car Repair Estimate or disconnect notice (if applicable)
