Visit the website to download and complete the registration form which can then be emailed back to [email protected] or delivered in person to 15655 South Ravinia Avenue Orland Park, IL 60462. Once registered, call (708) 403-6355 to schedule a ride. Basic trips required at least 24 hours notice. Medical-related trips require at least 48 hours notice. Same day requests may be taken depending on availability
