Eligibility

Person must:
  • Live in and receive dialysis treatment for end-stage renal disease (ESRD) in the U.S. or its territories
  • Be enrolled in health insurance coverage
  • Show that they cannot afford their health coverage, including: have a household income that does not exceed 500% of the Federal Poverty Level and have liquid assets that do not exceed $30,000
