AIDS Foundation of Chicago's supportive housing programs are collaborations among community-based social service providers that concurrently address the medical and housing needs of people who are chronically ill (with HIV or other chronic diseases) and homeless. The project partners believe that when homeless individuals with chronic medical conditions can focus their energy on healing and maintaining their health, rather than dealing with the hardships of living on the streets and in shelters, their health will improve.
