Case/Care Management | Camden County Care Coordinator

Provide Care Coordination to older persons and their family caregivers, including assessment, care plan development, monitoring and assistance in accessing public and private services. Provide assistance with Medicare Part D and Missouri Rx application and enrollment process. Can also advocate for clients through case/care management to those who are experiencing problems with those programs. AKA CCCC
