BEDS Plus Summit Service Center operates a drop-in center for individuals in the south and southwest suburbs of Chicago experiencing homelessness. This includes case management, financial assistance, and other wrap-around services to help transition individuals back into permanent housing.

Aunt Martha's provides homeless prevention services to help individuals and families stay in their current homes, shorten the amount of time they stay in shelters, and find affordable housing. To prevent or shorten the experience of homelessness, they may be able to help individuals and families pay their rent, mortgage, and/or utility bills. Their case managers are trained to support people and families, answer questions, and make sure you have a place to call home.
Aunt Martha's provides homeless prevention services to help individuals and families stay in their current homes, shorten the amount of time they stay in shelters, and find affordable housing. To prevent or shorten the experience of homelessness, they may be able to help individuals and families pay their rent, mortgage, and/or utility bills. Their case managers are trained to support people and families, answer questions, and make sure you have a place to call home.
Housing Forward works with individuals and families who are unsheltered and help them transition to supportive housing. Street Outreach Specialists connect with individuals by addressing basic needs such as food, water, personal hygiene kits, and clothing. As trust and relationship is built, staff members assess the needs of unsheltered individuals and connect them to case management, supportive services, and referrals.Diversion services help individuals and families who are facing homelessness to avoid emergency shelters or living on the street. This program provides immediate case management and support to help households rely on their existing strengths and support network.

Heartland Human Care Services' Supportive Services for Veteran Families (SSVF) program is funded by the United States Department of Veteran Affairs and is administered through Heartland Human Care Services to provide supportive services for low or very low-income veteran families residing in or transitioning to permanent housing. The services are designed to increase the housing stability of veterans and veteran families who are at risk of or experiencing homelessness. Homeless veterans (and families) are placed into scattered-site apartments in the city of Chicago or Cook County with independent landlords. The SSVF program is able to provide temporary financial assistance directly to the landlord based on the needs of the veteran household. Participants at risk of homelessness are provided with financial assistance to ensure they can remain in their current housing (usually rental payments). All participants are offered employment services and receive intensive case management, housing location/inspection services, and referrals to other services such as clinical, education, and legal services. This program serves 220 veterans and their households annually.

Assists homeless veterans and their families that are at a very high risk of homelessness. Community-based housing is provided for an average of 9-24 months. Supportive services include employment assistance, intensive case management, 3 months aftercare, substance abuse and mental health screening, and referrals to community resources (capacity of 25).
