Offers a variety of home based services dependent on individual personalized plans and needs. Available home based services include:

- Staff assistance with teaching various daily living skills.

- Transportation to medical appointments and other community resources.

- Accessing recreational opportunities in the community.

Provides a public bus service in DeKalb. Bus routes and schedules available on website.

Paratransit available for people with disabilities and who are unable to use regular bus service. Both points of destination must be within the DeKalb Urbanized Area (map online).

Non-Emergency Medical Transportation (NEMT) program: Transportation to medical appointments within 35-mile radius of DeKalb.

Provides one ride per week to any registered Champaign County resident for medical appointments, financial errands, grocery shopping, or personal errands. Two rides may be provided for those receiving dialysis or other physician ordered therapies.

Rides must be scheduled two days in advance.

Public transit service anywhere in Grundy County. Service to Joliet three times per day, including multiple different destinations. Includes paratransit services.

Transportation for seniors over the age of 60 and disabled individuals to medical appointments. Transports qualified residents up to 20 miles from the Township building to a medical appointment.

Offers transportation to individuals ages 60 and older for out-of-area medical appointments. Must call the office at least 24 hours in advance to schedule a ride. Donations are suggested for this service, but no one age 60 or over will be refused a ride due to inability to pay.

Provides free transportation for seniors for doctor appointments, dialysis or treatments, pharmacy or grocery store visits, activities at nutrition sites and senior centers, and more.

Transportation services only provide one (1) round trip per week or two (2) one way trips per week.
