It serves as a government- issued ID, a Chicago Public Library Card, a Chicago Transit Authority Ventra Card, and a Chicago Rx prescription drug discount card.
Cook County Clerk's Office provides birth records free of charge for verified homeless, domestic violence victims, and formerly incarcerated.
Helps you change your legal name and update your government identification documents. Our advocates will guide you through the process and paperwork, and then cut you a check for the entire cost. Applicable for: - Passport. - Driver's License and State ID. - Court Order Name Change. - Immigration Documents. - Tribal IDs. At this time we are not able to offer financial assistance for: - Birth Certificates. - Medical needs or procedures. - Housing.
Provides a case manager to work one-on-one with McHenry County residents who are homeless, in an effort to ensure that their health and safety needs are met. The case manager assists with essential services such as: - Obtaining vital records. - Signing up on housing waiting lists. - Obtaining medical and psychiatric services. - Locating emergency shelter and food. - Obtaining a mailing address and a phone. The case manager also assists those who are at imminent risk of homelessness by attempting to help them find what they need to stay in place.

- Coordinated Point of Entry: Acts as a centralized hub for all people experiencing homelessness in McLean County. During business hours, callers experiencing homelessness should typically be transferred to Homeless Services. Homeless individuals and families may also be directed to Coordinated Entry to be placed on a housing-related prioritization list. - Outreach for the Homeless: Provides assessment and linkage for people who are HUD homeless. Administers funds for direct services through case managers at the local shelters. Helps in obtaining a birth certificate and ID cards for clients seeking a clearance card in order to stay at one of the local shelters. Can sometimes assist with relocations, bus tokens for medical appointments, and homeless verifications. Completes VI-SPDAT housing assessments with clients in order to add them to the Prioritization List. - Motel funds may be available to homeless families when no other emergency shelter can be found.

Works to fill the “gaps” to help individuals and families overcome barriers to exiting homelessness, including: - Completing the Coordinated Assessment. - Obtaining identification. - Employment. - Education. - Health. - Other related expenses that resources are otherwise unavailable in the community. - Identifing housing. Limited services may be available to help imminently homeless individuals obtain or retain a rental unit.

Helps guests get State ID's and birth certificates. When resources are available, yearly bus passes may be purchased for guests who demonstrate a need. Bus Passes: Applications accepted on randomly selected days between the 20th and 25th of each month.

Other major DMV functions include: - Recording ownership (certificate of title) of the vehicles DMV registers. - Maintaining driving records (accidents and convictions) of licensed drivers. - Issuing identification cards for individuals. - Registering and recording ownership of vessels. - Licensing and regulating driving and traffic violator schools and their instructors. Also provides a service email for residents who have a driver’s license suspension or revocation or any other loss of driving privileges.
