White Crane's Health Outreach Program strives to provide a comprehensive, holistic, community-based approach to chronic disease prevention in order to reduce the incidence of risk factors for preventable disease (stroke, heart disease, cancer, hypertension, diabetes, pneumonia, flu, osteoporosis, mental illness); to motivate and educate seniors to take control of their health; and to make health care services accessible to low-income, disadvantaged, immigrant, refugee, and minority at-risk elderly.

Fall Prevention Programs
Program provides blood cholesterol tests, lipid tests, diabetes tests, height and weight measurements, health coaching, and information to lower heart disease risk to qualified participants. Participants must earn less than 250% of the Federal Poverty Level to qualify.
Provides those with diabetes the information they need to live active and healthy lives. Provided information includes self-care for optimal blood sugar control, classes and/or individual appointments that educate on diabetes management, ongoing diabetes education, nutrition counseling, and early identification of possible complications.
