Christian Community Health Center (CCHC) is a Federally Qualified Health Center (FQHC) that offers a comprehensive Hepatitis C program to treat and cure patients with Hepatitis C. The program includes screening, education, medication adherence, treatment, and prevention. Their clinical staff is specially trained in Hepatitis C management. Their team consists of an Infectious Disease provider, Pharmacists, Patient Navigators, Nurse Practitioners, and Medical Assistants. Their staff is dedicated to treating Hepatitis C and will assist patients in navigating the insurance process for a successful treatment outcome.
Free STD, HIV, and Hepatitis C testing is available both at the Rain office and through outreach events in the community. Pre- and post-test counseling, including prevention education, discussions of risk reduction methods, and referrals to needed services occur each at testing session. Community members who choose not to get tested can still access information and referrals.
Offers free initial testing for Hepatitis.
Provides Hepatitis A immunizations.
Alliance Care 360 (AC360) offers sexual health wellness services for all members of the community across Chicago and the Cook County suburbs. This short-term program provides sexual health and HIV/STI risk reduction education, medical appointment retention, and medication access support to anyone who is eligible at all of the Erie locations. Hepatitis C Services includes medical care, HepC treatment, which can lead to cure of HepC and referrals for specialty care and in-patient care.
TPAN offers free and confidential rapid HIV and hepatitis C (HCV) testing on a walk-in basis at their office and outreach in the community via a mobile testing unit (MTU).
