Works with healthcare providers, schools, long-term care facilities, and other partners to monitor for communicable diseases, including vaccine-preventable diseases. Communicable Disease: - Receives disease reports for communicable diseases and conducts contact investigations. STI Prevention and Control: - Receives disease reports for sexually transmitted infections (STIs/STDs) and conducts contact investigations. - Provides prevention education and resources to the community. - Assists impacted individuals with notification of partners. - STI counselor provides understanding about infection and links to treatment. HIV Case Management with Illinois HIV Care Connect: - Provides case management and supportive services to those living with HIV. Services for the Public: - Free condoms. - Testing and treatment for HIV and STIs (HIV Care Connect - medical case management, testing and treatment, assistance informing partners and getting them in treatment, PrEP). - Guidance and resources on: preventing communicable disease, handwashing, immunizations/vaccination, when and how to isolate or quarantine when needed, what to do if a bat is found in house or individual is bitten by a wild animal. - Preventing STIs/STDs (request a sexual health presentation, screening and treatment). - Common communicable diseases including vaccine preventable diseases (request a presentation on a current disease). - Staying healthy when traveling.
