Assists in locating a medical home for patients who have visited Bryan East or Bryan West Emergency Departments in Lincoln for health care issues. Program staff will help find a provider, assist in obtaining needed medications, and connect patients to other community resources. The goal of the program is to connect individuals and families to a total health care program for healthier lives. Patients will meet with a registered nurse/case manager team at a time and place that works for them. The case manager works with the patient to find a medical home (i.e., primary doctor) and access medication resources.

Medical Expense Assistance

Administers the Volunteer Physician Program (VPN) that provides specialty care for Central Iowans who are uninsured, underinsured, or undocumented, whose income level is at or under 200% of the federal poverty guidelines, who are not eligible for any government funded programs. This program may be accessed through over 54 Safety Net and Free Clinics of Iowa, Primary Health Care and Proteus by referral, after a patient is financially vetted to qualify financially for this program and not be eligible for any other program.
