Patients receive FREE blood pressure monitors to encourage them to check their blood pressure at home regularly. A CFHC clinical team monitors their readings remotely and provides proper instructions, information, and resources.
The Illinois Pathways to Health is an initiative designed to connect people with programs to increase knowledge and teach skills to better manage chronic conditions, reduce fear and risk of falls and live happier, healthier, more independent lives! The programs are comprised of highest tier evidence-based small group-interactive workshops.
