BBO-C is a statewide program (outside the City of Chicago) that provides pregnant and parenting clients with direct, comprehensive, nursing assessments and assistance to connect and engage with desired services and understand the importance of a medical home.

Nurse Navigator's provide nursing assessments and any immediate nursing interventions necessary to dyads (pregnant and postpartum individuals and their infants), helping to connect families to the needed supports available in their community during in-person visits. Nurses are required to conduct the initial pregnancy visit and initial postpartum visit for this program and are expected to follow up as indicated using clinical discretion. Non-nurse navigators may provide follow-up at the discretion of the nurse for collaborating with each family to identify and mitigate barriers to accessing desired supports and services.
