Christian Community Health Center (CCHC) offers primary medical care for the whole family, at every stage of life. Their doctors and medical staff are dedicated to providing comprehensive, high-quality, and affordable evidence-based medical care.
PrimeCare Health Community Health Centers are Federally Qualified Health Centers (FQHC) that provide comprehensive primary healthcare services to the community. Patients without insurance will be assisted in enrolling for services.

Follow-up visits for patients with diabetes, elevated blood lead levels, hypertension, behavioral health concerns and other on-going health problems are also routine activities at the clinic.

Health education integrated into each patient's visit. Nutrition, disease prevention and management are taught to patients individually and in groups. Educational follow-up, care coordination, management and peer education, including care for sexually transmitted diseases, latent tuberculosis, high lead levels, prenatal care and care for chronic diseases (asthma, diabetes, hypertension, high cholesterol, and depression).

Specialist referrals for services not available at the clinic are made for all patients. Appointments for patients without insurance and/or limited income are made through Hope Medical Outreach Coalition volunteer physicians and facilities.

Heartland Alliance Health is a federally qualified health center (FQHC) with clinics located in Uptown and Englewood. More than 75 percent of participants are homeless, many living with multiple chronic mental and physical illnesses. HAH also delivers care at shelters and community settings through medical outreach teams. HAH provides participants with preventative care, urgent care for acute illness, and ongoing primary care and disease management for chronic conditions.
Provides immunizations, communicable disease follow-up, environmental inspections, certified copies of birth and death certificates, emergency planning, health safety education and school health.
