Provides care coordination, advocacy and support for individuals and families. Team helps to manage whole health including physical, behavioral, and social health services.

Offers individual, couple, and family psychotherapy, as well as psychoactive medications management and monitoring, and Integrated Home Health (IHH).

Community Support Services (CSS) provides workers in the individual's home and community to help overcome barriers to mental health and healing. Monitoring of mental health symptoms, crisis planning, and support to build natural support systems that help clients get through their day.

IHH brings together mental, physical and social health services into one integrated program. Each family is connected to a three-person team that includes a nurse, a care coordinator and a family peer support specialist who provide flexible support focused on individual and family strengths, needs and comfort level.

The mental health team works with patients to assess current coping skills, address emotional needs, and work through lifelong patterns. A board-certified psychiatric mental health nurse practitioner provides specialty care in the areas of psychiatric consultation, evaluation, and psychiatric medication management. These services are provided on-site by the mental health team.

Provides a comprehensive and integrated range of mental health, substance use, and primary health services to any individual, regardless of funding. Integrates mental health and substance use care with physical health care, utilizing a wide variety of evidence-based practices. Responsible for directly providing (or contracting with partner organizations to provide) nine required types of services: crisis mental health services; screening, assessment and diagnosis, including risk assessment; patient-centered treatment planning; outpatient mental health and substance use services; primary care screening and monitoring of key health indicators/health risk; targeted case management; psychiatric rehabilitation services; peer support and family supports; and intensive, community-based mental health care for members of the armed forces and veterans.

Offers the FACT (Forensic Assertive Community Treatment) program, which provides services to individuals in the criminal justice system with a serious mental illness. FACT works to help people live successfully in the community while avoiding a return to jail, prison, or unnecessary psychiatric hospitalizations. Services are provided 24 hours a day, 7 days a week. The program focuses on person-centered treatment planning and operates as a mobile outreach team - meeting clients in the home or community to provide services.
