Esperanza Community Services has a variety of case management opportunities. Their Client and Family Support Program helps families deal with legal, medical, and other social issues. This service is offered in the family's home. The program helps individuals with developmental disabilities reduce behaviors that create a barrier to learning and optimal community integration (e.g., outbursts, self-isolating, self-injuring) and increase positives ones (e.g., appropriate social interactions and communication, staying on task, and life skills).

Immanuel Home Care Services (IHCS) provides coordinated care of medical, social, educational, vocational, and other services related to the person's health needs and information to help better achieve the goals of treatment and care.

New Choice Intervention empowers and promotes the wellness of clients through effective day-to-day oversight and administration of the client's service plan. Client-centered case management services are designed to help clients and their family members gain access to, and benefit from, available Waiver and Medicaid State Plan services, including medical, social, and educational services within the community. Effective referral linkage and support services are provided to promote a client's independent lifestyle.

Daughter of Destiny Outreach, Inc. helps support wounded women so they can be restored back to wholeness. Services include: 1:1 sessions, group processing, recovery coaching, mentorship, case management, spiritual support, basic life skills, parenting classes, home economics, and workshops.

Elara Caring - Des Plaines coordinates clients' access to social services such as housing, food, medical care, and more.

Catholic Charities Regional Services bring services closer to people in communities throughout Cook and Lake counties. Each Regional Office can meet emergency needs with food, clothing, referrals, and other assistance. Additionally, the Regional Offices facilitate other vital programs for individuals or families including counseling and housing assistance.

SER/SERCO locations offer educational, emotional, and career support to young adults, ages 16-24, living in low-income, underserved communities, especially those who are ex-offenders or gang-affected. Services include after-school programming, supplemental education, GED classes, job and internship skills training, education pathway planning, career coaching, financial literacy, mentorship, summer employment referrals, case management referrals, transportation, service learning projects, and leadership training.
