Esperanza Community Services has a variety of case management opportunities. Their Client and Family Support Program helps families deal with legal, medical, and other social issues. This service is offered in the family's home. The program helps individuals with developmental disabilities reduce behaviors that create a barrier to learning and optimal community integration (e.g., outbursts, self-isolating, self-injuring) and increase positives ones (e.g., appropriate social interactions and communication, staying on task, and life skills).

National Association for Down Syndrome delivers baskets of gifts and information for families at hospitals.

JenCare Senior Medical Center provides specialty health care services to the senior community. Transportation to and from appointments is available by request. Pharmacy services are available on-site.

Nourishing Hope offers services designed to support and empower by partnering with clients to tailor a plan to reach their own unique goals. They assist clients in making changes in their life by navigating the resources that are available to them such as public benefits, transportation, financial, housing, mental wellness, and employment. Virtual case management and mental wellness sessions by phone call, video call, or email are available.

MYSI Corporation facilitates the coordination of medical and social services on behalf of seniors.

Chicago Treatment and Counseling Center II provides referrals for individuals needing counseling, testing, or treatment for HIV/AIDS and Hepatitis C.

Claretian Associates facilitates arranging for services that assess the individual's needs and coordinate the delivery of necessary social services.

New Age Services Corporation (NASC) - Chicago facilitates the coordination of medical and social services on behalf of an individual or family.

Children's Place Association - Arthur E. Jones Early Childhood Care and Learning Center facilitates the coordination of medical and social services on behalf of an individual or family.

New Choice Intervention empowers and promotes the wellness of clients through effective day-to-day oversight and administration of the client's service plan. Client-centered case management services are designed to help clients and their family members gain access to, and benefit from, available Waiver and Medicaid State Plan services, including medical, social, and educational services within the community. Effective referral linkage and support services are provided to promote a client's independent lifestyle.

Suburban Access - Homewood provides service coordination and advocacy for individuals who have intellectual disabilities, reside in skilled nursing facilities, and were involved in a class-action lawsuit.

The Home Visitors Program was developed by Near West to strengthen and promote personal and economic self-sufficiency for families transitioning and/or living in the West Haven community. All services are confidential. Services includeCase Management, Budget Assistance, Economic Empowerment, First-time homeowners information, Home Management and Housing Advocacy, and Individual and Family Counseling.

Healing, Empowering & Learning Professionals (H.E.L.P.) LLC offers individual, couples, family, and group therapy to adults and youth. They use culturally responsive techniques to provide support and coping strategies for many areas of mental health including, but not limited to trauma, internalizing disorders (e.g., depression, anxiety), externalizing disorders (anger, attention deficit/hyperactivity), relationship difficulties, and more.

The Link & Option Center, Inc. offers a variety of mental health services including: psychiatric evaluation, mental health assessment and treatment, treatment for substance use disorders, and comprehensive case management.

Oak Park Township strives to promote the well-being of all its older residents. They offer a wide variety of affordable, and often free, services geared toward enhancing and improving the quality of life for senior residents including the chore program, affordable wills, benefits assistance, a memory cafe, the senior health insurance program (SHIP), caregiver support, and the home repair program.

Village of Skokie facilitates coordination of medical and social services on behalf of an individual or family. The Social Work staff provides information and referrals for such needs as community resources, employment resources, home care providers, housing resources, financial assistance providers and availability, legal assistance resources, and retirement/nursing home resources. The social workers provide advocacy and short-term counseling for personal difficulties, family problems, and other life issues. Long-term counseling needs are referred to outside agencies. The social workers will make a home visit if requested.

Village of Arlington Heights offers crisis intervention, assessments, brief therapeutic intervention, and provision of mental health referrals. Additionally offered is the Counseling Subsidy program which provides a sliding scale subsidy for counseling services with partnering community agencies or private practices.

UI Health - Child and Youth Center facilitates the coordination of medical services on behalf of pediatric patients.

Cook County Department of Public Health (CCDPH) Clinics - North Suburbs offers a Family Case Management (FCM) program that provides comprehensive service coordination to improve the health, social, education, and developmental needs of pregnant women, and infants (0-12 months) from low-income families in the communities of Illinois. The program connects an individual with a medical provider while helping one to understand the importance of regularly scheduled well-child visits and immunizations. Other resources the program services can connect an individual to are family planning, WIC, health education, hearing/vision, substance use support, parental/parenting classes, child care, housing, smoking cessation, lead screening, transportation, early intervention, immunizations, well-child exams, All Kids (health insurance) and domestic violence support.
