Jones Memorial Community Center facilitates coordination of medical and social services on behalf of an individual or family.
DUI Community Counseling Center facilitates the coordination of social services in order to access food, housing, or employment.
Village of Skokie facilitates coordination of medical and social services on behalf of an individual or family. The Social Work staff provides information and referrals for such needs as community resources, employment resources, home care providers, housing resources, financial assistance providers and availability, legal assistance resources, and retirement/nursing home resources. The social workers provide advocacy and short-term counseling for personal difficulties, family problems, and other life issues. Long-term counseling needs are referred to outside agencies. The social workers will make a home visit if requested.
Henry Booth House provides outreach and coordination of medical health, oral health (dental), and social services for families with a child enrolled in the Head Start program.

Additionally offered is the Counseling Subsidy program which provides a sliding scale subsidy for counseling services with partnering community agencies or private practices.

Healing, Empowering & Learning Professionals (H.E.L.P.) LLC offers individual, couples, family, and group therapy to adults and youth. They use culturally responsive techniques to provide support and coping strategies for many areas of mental health including, but not limited to trauma, internalizing disorders (e.g., depression, anxiety), externalizing disorders (anger, attention deficit/hyperactivity), relationship difficulties, and more.

The HUB provides assistance from professional staff with comprehensive information and direct connections to HIV services for people living with and vulnerable to HIV. Individual assessments will identify specific needs and connection to appropriate resources such as HIV/STD testing, PrEP support, primary care, housing assistance, medical transportation, emergency financial assistance, behavioral health and case management services and other essential services.
Suburban Access - Homewood provides service coordination and advocacy for individuals who have intellectual disabilities, reside in skilled nursing facilities, and were involved in a class-action lawsuit.

Esperanza Community Services has a variety of case management opportunities. Their Client and Family Support Program helps families deal with legal, medical, and other social issues. This service is offered in the family's home. The program helps individuals with developmental disabilities reduce behaviors that create a barrier to learning and optimal community integration (e.g., outbursts, self-isolating, self-injuring) and increase positives ones (e.g., appropriate social interactions and communication, staying on task, and life skills).
The Home Visitors Program was developed by Near West to strengthen and promote personal and economic self-sufficiency for families transitioning and/or living in the West Haven community. All services are confidential. Services includeCase Management, Budget Assistance, Economic Empowerment, First-time homeowners information, Home Management and Housing Advocacy, and Individual and Family Counseling.
