Illinois Action For Children offers Home Visiting for pregnant women and families of infants, toddlers, and twos. Parent educators will partner with you, in your home (or one of the Early Learning Centers), in three key areas: Child-centered parenting, Parent-child interaction and Family well-being.
CCAC provides free home visitation, support groups, and supportive services to income qualifying pregnant women and parents with children from birth to age three living in and around the Roseland area.Families are visited in their home by a parent coach who support parents as their child's first teacher. Parents learn about healthy child development and practice parenting techniques to enhance bonding and family well-being. Pregnant women are provided support and resources as they prepare for the birth of their child.

Family Solutions for Kids: Provides in-home intervention with families in their own homes, building on strengths and sharing important life skills to help keep them together.
Parents as Teachers: Parent-child early education program for children through kindergarten entry. Services includes personalized visits, group meetings, developmental screenings and referrals services.

The Parent Empowerment Program (PEP) promotes early learning, brain development and resiliency in young children ages 0-3, empowering parents to be their child's first teachers. The Educators bring activities from evidence-based curricula that strengthen the parent-child relationship and help kids achieve developmental milestones.
Parent education for families prenatally to age five (before kindergarten). Yearly developmental screenings, home visits and group meetings are conducted with the families. Referrals to other resources are made if necessary.

Healthy Families Illinois: Provides in home services to first-time parents and their newborns focusing parent-child interaction, child growth and development. Also promotes child well-being and prevention of abuse and neglect. No face to face contact due to COVID-19.
Home Visiting Program: Offers home safety checks, developmental screenings, literacy training and free books, parent education, playgroups, resource network in times of need and help for parents to cope with stress.
Provides personal visits that inform parents about their child's current and future development and offer ideas for developmentally appropriate activities.
Home visiting services for families who are expecting a baby or have children ages 0-5. The program is free and voluntary and provides support to help families become self sufficient.

Provides voluntary home visiting services that offers family support and coaching for expectant parents and parents of young children ages birth to 3. The program provides guidance about child development, child safety, practical parenting skills, and family bonding. Provides regular visits based on the family's needs, preferences, and schedules. Assists pregnant women and families with children under the age of two to prepare for a new baby and to provide the best care possible after that baby arrives and as he or she grows. Services will focus on prenatal care, child development, child health, parenting skills and other aspects of family functioning. In-home nursing services are available to pregnant women and infants with medical needs.

A home visitation program that provides parent education, family support, early intervention services and serves as a link to health care services.

Provides therapeutic and skill-building interventions within the home. Families referred may have severe mental health issues, abuse or neglect issues, or youth with serious emotional and behavioral disturbances. The major goal of this program is to prevent an out-of-home placement of the youth to reunify families and stabilize placement. Omni provides this service to preserve the family's integrity and prevent unnecessary out-of-home placements, to put adolescents and their families in touch with community resources for long term outside support, and to strengthen the family's coping skills and capacity to function effectively in the community after treatment is completed.

WIC Clinic is open the 1st, 2nd, and 3rd Tuesday of each month. Eligible individuals can receive nutrition education, referrals to community services, and vouchers for nutritious foods. Additionally, Peer Educators are available to provide breastfeeding education and support to pregnant and postpartum women enrolled in the Nebraska WIC program. Provides a program that pairs families with a home-visitor based on each family's needs and schedules. Home visitors emphasize practical parenting skills as well as family bonding before birth and as children grow up.

- Home Health Aide: Provides individuals an opportunity to maintain safety and independence in their home environments, delaying or preventing the need for transitions into a higher level of care such as a skilled nursing facility.

- Parenting Support: Offers home visits, educational opportunities, and community referrals so parents are empowered to maintain self-sufficient, healthy, and happy families.

- Shelter Nursing: Nurses visit homeless clients at shelters, crisis centers, and through outreach on the streets. Provides health screening, assessment, and referrals with the intent to ensure individuals and families residing in shelters have access to comprehensive health, community, educational and social services.

- Flu and Immunization Program: Provides immunizations clinics in the community, corporate sites, senior living centers, and drive through in Omaha, Lincoln, Council Bluffs, and surrounding communities.

- School Health: Provides health screening and assessment, referral, and follow-up; immunization compliance; physical and vision exam compliance (kindergarten, seventh grade, and new students); development of student health and emergency plans; communicable disease control; handling of sudden illness and injury; training of first responder teams and those giving medications; and health policy development.

Provides a free program for families with kids, ages 5 and younger, living in Dubuque County. At personal visits, children learn through play with provided materials and families get to connect through activities, games, and crafts. Families get information about every stage of child development and are invited to monthly groups to connect with other families.

Also offers annual screenings to make sure kids are developmentally on track and connect families to community resources. PAT partners with parents and caregivers to support them through their parenting journey.

Parenting Materials

Family resource program to support parents in educating and nurturing their children. The program provides parents with opportunities to move toward self-sufficiency by working with other community organizations. Health and mental health services are also provided to participants.

Program helps parents better understand their child's stages of development with individualized visits in the home. The weekly visits are 90 minutes in length and are designed to meet the needs of each family. Staff will assist families in planning activities that encourage the child's growth. Social activities will be held twice monthly to provide the opportunity to interact with your child and other families.
