A mobile unit offers emergency food, clothing, referral sources and support to homeless youth, runaway youth or youth in crisis.
Homeless outreach services that help with accessing resources and develops programs that fill gaps and meet unmet needs in outreach, housing and education.

Provides community and street outreach services to assist individuals who may be facing homelessness by working to increase job, social, and living skills. Will also assist individuals with obtaining benefits (Social Security, Medical, etc.) for which they may qualify.

Through Good Neighbors Field Outreach, teams of staff and volunteers deliver toiletries, clothing, and food to individuals experiencing homelessness or at risk of experiencing homelessness. On various days during the week, teams connect with people residing on the streets in the Near North neighborhood to listen, hand out daily living essentials, and direct people to the resources they may need.

Provides street-based aid to youth experiencing homelessness.

Includes:

- Survival aid, education, and connection to emergency shelter

- Shelter and habitations visits seeking out youth who are homeless and who need assistance with job attainment, re-entry to school, connection to GED or vocational programs, or obtaining housing

- Individual counseling at the youth's site or at the Project Oz office

The Nonviolence Chicago Street Outreach teams advocate for the community. They provide food and toiletries and conduct well-being checks when needed. To help communities take back space where violence has occurred, they host events creating safe spaces and building relationships with neighbors.
Compassion Day: Prepares and distributes non-perishable foods, hygiene and seasonal supplies to the homeless living on the streets.
Provides the following for homeless individuals in the area: - Food (meals and snacks). - Toiletries. - First aid. - Underwear. - Socks. - Winter coats. - Blankets. - Connection to community resources.

Provides basic needs items, referrals for services, and connection to shelter to runaway and youth experiencing a housing crisis by meeting them where they are both physically and psychologically.
Mobile Outreach Unit: Goes out in the community to provide the homeless with resource assistance, counseling, referrals, etc. No pickups after 5:00.

Assists the needs of people living on the streets (outside, abandoned buildings, in cars, etc.) in the Omaha Metro area who are interested in connecting with housing and/or other services.

Services provided are:

- Screening and referral for housing programs

- Information and referrals for mainstream benefits (SNAP, Medicaid, GA, etc.)

- Emergency supplies (food, water, blankets, hygiene items)

- Connection to emergency shelter, if desired

- Case Management

- Basic health assessments and connections to long-term mental and primary healthcare

Rapid Re-Housing Programs
The Thresholds Homeless Outreach Program was established in 1989 to provide assertive outreach, linkage to primary and psychiatric care, and resource referral to persons experiencing homelessness with serious mental illnesses. The program employs the skills of licensed clinical social workers on the streets, in shelters, and in any other location where people who are homeless may be found.
The Street Medicine Program brings free health care, survival supplies, and support directly to individuals who have the most difficulty accessing traditional services. Regularly visiting encampments, expressway viaducts, and street corners across the city, a Nurse Practitioner, Case Manager, and Street Medicine Outreach Professional first address immediate health needs, then link individuals to case management and medical homes.
Ignite's SCOPe (Street and Community Outreach Program) offers support to youth ages 14 to 24in the south and southwest areas of Chicago to build relationships with young people experiencing homelessness, connect them to community resources and the full array of Ignite programs, meet their immediate needs, and provide crisis intervention.
LPCS provides weekly outreach and drop-in supportive services (showers, laundry, clothing, toiletries, meals, referrals, and case management) to individuals who are currently experiencing street-based homelessness.
Drive around St. Louis looking for individuals who appear homeless, and provide them with clothing, food, and essential items.
YOT's Outreach Case Managers connect with clients across the city and link them with safe housing, education, employment, and other resources as well as providing compassionate support. In addition to making a regular stop with the Youth Outreach Van in Chicago's Lakeview neighborhood every Thursday evening, Team members visit libraries, courthouses, coffee shops, and other locations where young people experiencing homelessness often congregate.

The Project for Assistance in Transition from Homelessness (PATH) Program, funded by the Illinois Department of Human Services, Division of Mental Health, provides community-based outreach, mental health, substance abuse, case management and other support services, as well as a limited set of housing services.
Northwest Compass services include intake and referrals, crisis intervention, solution-focused case management, street outreach, individual financial counseling, and money management classes, career coaching and job readiness, and more to alleviate hardships and allow clients to become stable and stay empowered in their everyday lives.

Habilitative Systems, Inc. provides community-based outreach, mental health, substance abuse, case management, and housing services. Services include screening, assessment, linkage, and referrals to appropriate levels of care for homeless adults. The program's goal is to reduce homelessness, and recidivism and stabilize support systems.
Featherfist, Inc. offers street outreach specifically on the south side streets of Chicago. They work to form relationships that can help break the cycle of homelessness.
