The Chicago Community Health Response Corps offers an information and referral hotline service where community members can call to inquire about local resources related to COVID-19 vaccination and testing, housing assistance, food, medical needs, pet needs, and more. The Chicago Community Health Response Corps is not a direct service provider. Interpreter services are available to callers who speak a language other than English or Spanish.

Offers an assessment of needs and connects them to local resources, such as: - Home delivered meals or community dining programs - Benefits programs to help pay for prescription drugs, health care, rent, utilities, and other needs - Family Caregiver Support for those who are giving unpaid care to adults with Alzheimer’s disease or a related disorder. - Transportation services (doctor appointments or personal errands) - In-home assistance ( light housework, personal care, and meal preparation) - Community engagement activities Also provide advocacy and follow-up to see that the services were received.
