CDC-INFO: Provides information and referral on more than 750 health and safety topics, including COVID-19, HIV/AIDS, Sexually Transmitted Diseases (STDs), Ebola, Zika, Seasonal flu, immunizations, reproductive health/maternal mortality, smoking/tobacco use.
Maintains information on a wide variety of public health hazards and topics, including various diseases and conditions (such as cancer, COVID-19, influenza, diabetes), food safety, wellness and prevention (smoking cessation, immunizations) and opioids.
Information & referral services and a support helpline.
Provides telephone information concerning communicable disease.

Women's Health Line: Available to answer questions about women's health issues, help callers locate clinics providing breast/cervical cancer screenings, information on sexually transmitted diseases and resources for treatment of eating disorders. Brochures and fact sheets about women's health issues are available on request.

Offers an information line operating 24 hours a day, 7 days a week. Provides health information, triage, physician and service referrals over the phone for individuals in need of medical advice.

Support groups offered through Iowa Health Systems: for example, breast feeding education & support group, parents of children with ADD, grief support for parents whose child died, and others.

The Care for Kids Program - an Early Periodic Screening, Diagnosis & Treatment (EPSDT) program - serves residents of Polk and Dallas Counties. EPSDT is the benefit package of Medicaid/Title 19 for individuals under 21 years of age.

The focus of the EPSDT Care for Kids program is to inform families of the benefits they receive with Medicaid/Title 19. -help individuals secure a medical and dental home. Ensure that eligible individuals receive and understand the importance of preventative health care. Link families to other community resources to improve their health and well-being (WIC, Early ACCESS, parenting classes, child care assistance, housing assistance, food pantries, etc.).

Maternal Health provides information about what to expect during pregnancy, including how to deal with physical and emotional changes and advice to prepare for the arrival of the new baby. Can also provide vitamins and help with locating health care providers.

Child and Adolescent Health coordinates free dental services, including screenings and fluoride varnishes. Can also perform blood lead testing and provide information and referrals to health and dental care services in the area, including mental health.

Offers a Registered Nurse who provides technical assistance, guidance, community resources, support, and trainings to childcare providers and their families. The RN is able to answer any questions childcare providers or their families have pertaining to the development, social emotional and general health and safety of children in care.

Provides information and referral for mental health services for deaf, hard of hearing, and deaf/blind individuals in the state of Illinois.

A free public service to answer questions about cancer and help with smoking cessation. Program resources available to patients, family members or friends, health care providers and researchers.

Provides health information, what to do if you do not feel well, find a physician to meet your needs, can register you for classes, programs, health screening and support groups.

Provides a personal emergency response system that offers simple, fast access to help 24 hours a day.

The Population Health team is a free resource available to help patients reach their health and wellness goals.


Nurse Health Coaches work one-on-one with patients and their families to evaluate health needs, environmental factors, and other barriers to their health and wellness. They then help patients establish plans and goals to achieve a healthy lifestyle, working closely with local and state resources to ensure that each person receives the support that they need.
