Case/Care Management
Our Children's Healthcare Center provides care for medically fragile and/or technology-dependent children, ages newborn through 21 years.Most children are admitted from a hospital NICU or PICU. This program focuses on patients who no longer require acute care but need significant medical attention related to wound therapy, PICC antibiotics, post-surgical care, NG, GJ tube feedings, and ventilator care. The Center is a "home away from home" for medically fragile children and their families.

Subacute Care Nursing Facilities
Patient Advocate Foundation (PAF) facilitates the coordination of social services in order to access food, housing, or employment. Case Managers offer free, one-on-one support to patients and families to help them solve real-life problems related to a serious health condition. They help patients understand their choices to make healthy decisions for a strong recovery. PAF Case Management is free for patients and families living with serious or chronic health conditions.
