Investigates confirmed and suspected cases of reportable diseases by working with healthcare providers, schools, childcare staff and the Illinois Department of Public Health (IDPH). Collection and analysis of data from infected and exposed individuals (surveillance) to identify the cause of diseases to prevent and control their spread from person to person. Also, provides Tuberculosis (TB) testing and treatment, treatment for partners of those diagnosed with a sexually transmitted infection (STI) and HIV Medical Case Management.

Provides Adult Group Homes. Each individual actively takes part in the day-to-day activities of cleaning, cooking, shopping, along with a wide variety of recreational activities. Elmwood Heights: Residential facility for 67 adult individuals who are physically and developmentally disabled as well as medically fragile. A comfortable homelike atmosphere combining highly specialized services with around the clock medical care. Personalized attention and training is provided to help the individual develop daily living skills.

Provides services for seniors, including information and referral, senior health insurance program (SHIP), community outreach, recreational and wellness activities. 

Offers emergency financial assistance once per year to anyone in need. Assistance may be given in the areas of rental assistance, disaster relief, gasoline, utility (water/sewage/trash only), and uniforms assistance.

Provides access to needed resources through comprehensive outreach, information and referral services.

Provides the following community-based therapeutic recreational services:

- Athletics and Special Olympics.

- Social outings.

- Arts and crafts.

- Music.

- Day Camp.

- Adult Day Programs.

Provides on-site counseling and pastoral counseling that includes support groups. Can help with anger management, grief counseling, general counseling, and faith based counseling.

Gives away free items seasonally, depending on availability: 

• Winter coats and blankets (winter)
• Space heaters (winter)
• Christmas toys for children distributed by Santa
• Fans (summer)

Guides families through their child’s journey with a medical condition. Works with doctors, schools and community groups to create a seamless support system.

Care coordination team works together with families to develop a plan of care that addresses a child’s medical, social, behavioral, educational and financial needs. Care coordination is free for all children who have eligible medical conditions, regardless of their family’s income level.

Care coordination can include finding specialized medical care, helping families understand their child’s diagnosis and medical treatment plan, explaining insurance benefits and attending school meetings.

Helps children who are medically fragile remain in their family home rather than a hospital or skilled nursing facility.

Assists with:

- Finding specialized medical care

- Making sure families understand their child’s diagnosis and medical treatment plan

- Helping families understand their insurance benefits and maximize their coverage

- Attending IEP /504 Plan school meeting

- Connecting families with financial support, grants and other community resources

- Preparing for the transition to adulthood

Provides a food pantry to those in need who live in a Livingston County community.

Provides information regarding healthcare emergency preparedness plans and preparedness activities for the Greene County Emergency Response Coalition. Also provides personal and family emergency preparation information.

Provides toys and food for families in need during the Christmas holiday.

Offers free meals, daytime shelter, free laundry services, showers, and individual lockers to keep personal items secure.

Provides ramps and other modifications to increase accessibility for disabled homeowners and renters. For minor home repairs services include repairing condemned furnaces, faulty wiring, or actively leaking roofs. Major home repairs provide funds in the form of forgivable deferred loans to correct health and safety code violations, including lead-based paint hazards for single-family dwelling units located in low-income census tracts. Get the Lead Out program and Health Home Production awards housing funds to qualified applicants to help cover the cost of lead hazard remediation. This program is available to owner-occupied homes and privately-owned rental properties. 

Member-based organization for individuals and organizations interested in community issues related to refugees and immigrants. Meets unfulfilled needs of immigrants and refugees in the Quad Cities, such as english as a second language.

Can assist with applying for resources in Illinois, including SNAP, Medicare/Medicaid, SEAP, etc. Can help you apply for up to $500 to pay for DACA renewal fees.

Offers financial assistance to those that qualify.

Offers a clothing bank.

Assists homeless teens with a safe, stable home, provides income adequate to meet their basic needs, and assists and directs in overcoming the problems that interfere with their ability to live independently.

Provide a single point of entry to the Illinois Early Intervention Program (EI). Early Intervention offers developmental support and case coordination services for families with children under 3 years old with developmental delays. Service Coordinators assist referred families through the process of enrollment into the EI, as well as initiate referrals for evaluations and direct services. Eligible participants receive case coordinator through CFC to monitor individualized goals, strategies, and services. Additionally, staff assist families with the transition from the EI program into local school/care special education services based on the family's priorities and the child's needs.

Provides inpatient and outpatient mental health services. Services include treatment for depression, anxiety, trauma, PTSD, bipolar disorder, schizophrenia, self-harm and suicidality, family/relationship problems, OCD, and confusion/memory loss.

Provides individuals suffering from mental health diseases and/or chronic substance use disorders with peer-led sober living houses. House rules and structure are geared toward the maintenance of recovery. Three (3) recovery homes in total are available.
